Medication Management Services
Managing medications can be confusing, especially when you’re juggling multiple prescriptions, each with its own timing, specific instructions (such as whether to take with or without food), and varying methods of delivery like oral, injections or IV. At KompleteCare, we’re here to provide the support you need, offering two distinct types of medication management services.
Medication Assistance
This is ideal for you if you can take your medications independently but need a little help and support to stay on track. Medication assistance is designed to help you retain your independence and ensure your safety and includes
Medication reminders (reminders to take medications at the correct times).
Assistance with opening containers or blister packs.
Help with organising Webster Packs (pre-packaged blister packs containing your personal medication for the day or night).
Guidance on following dosage instructions.
Please note that this service does not include administering your medication.
Medication Administration
This is ideal if you have complex medication routines or if you need direct assistance to take medications. Whether it be intravenous, oral or topical medications our staff are trained and certified to assist you to ensure accurate and safe dosage at the prescribed times. Medication administration includes
Accurate measurement and direct administration of medications (oral, topical, or injectable).
Monitoring for potential side effects or adverse reactions.
Secure medication storage and safe handling practices.
Medication Management Plan
To ensure the highest levels of care and consistency, our team of registered nurses create a Medication Care Plan for all medication supported clients. A Medication Care Plan is a personalised roadmap for managing your medication needs safely and effectively. It is developed in collaboration with you, your care team, and your healthcare providers to ensure all aspects of your medication routine are tailored to your unique health needs.
Your Medication Care Plan Includes
A detailed list of all prescribed medications, dosages, and schedules.
Clear instructions for proper administration or assistance.
Monitoring protocols to track side effects or interactions.
Contingency steps for missed doses or emergencies.
Roles and responsibilities of caregivers or healthcare professionals.
NDIS Medication Management
NDIS Medication Management is typically funded through the Assistance with Daily Living category of your NDIS funding. However, if the support involves a higher level of intervention, such as assisting with the administration of medication (e.g., injections or IVs), it might be funded under Health and Wellbeing supports. Either way, our team is here to support you every step of the way.
Medication Management Aged Care
For Home Care Package recipients, medication management is typically funded under the Assistance with Daily Living or Personal Care categories of the Home Care Package. This funding can cover a range of medication-related supports, including
Medication reminders or prompting.
Assistance with taking medications
(e.g., administering medication in the right dose at the right time).
Medication management services
Including organising medications into blister packs or managing complex medication regimens.
